TOGETHER TOWARD TOMORROW: WEOHT PFAC VISIONING CONFERENCE SUMMARY REPORT 2025
On Saturday, September 27, 2025, the WEOHT Patient/Client, Family, and Caregiver Advisory Council (PFAC) hosted the Together Toward Tomorrow Patient, Family, and Caregiver Visioning Conference.
This half‑day event welcomed 83 patients, families, caregivers, healthcare professionals, and system partners, creating a vibrant space to celebrate lived experiences and strengthen collaboration across our health system. Guided by the Patient, Family and Caregiver Declaration of Values for Ontario, participants engaged in meaningful dialogue, shared learning, and co‑creation to help shape the future of patient‑centred care within the WEOHT.

Indigenous Cultural Safety Training
The WEOHT is providing Indigenous Cultural Safety Training Sessions this January through March. We welcome individuals working in the Social, Community or Healthcare services sector to register for a training session.
The session provides organizations with a meaningful opportunity to advance reconciliation efforts and create culturally safe environments. By deepening understanding of Indigenous histories, perspectives, and health equity, teams can begin to transform practices, challenge assumptions, and build stronger, more respectful relationships with Indigenous communities. The Windsor-Essex Ontario Health Team (WEOHT) is comprised of over forty healthcare organizations, community support services, hospitals, home and community care providers, lived experience partners, and primary care providers delivering services in Windsor-Essex to over 450,000 residents.
Ontario Health Teams are a new way to organize and deliver health services, bringing together multiple health are providers as one collaborative team to provide patients with better connected and more convenient care, closer to home.
