We are seeking a part-time 0.1 FTE Clinicial Lead (Primary Care) to be part of this dynamic Ontario Health Team
This position reports to the Steering Commitee for Windsor Essex Ontario Health Team (WEOHT).
JOB SUMMARY:
The Clinical Lead (Primary Care) works closely with key stakeholders in Windsor-Essex to support the evolution of system and resource integration within the WEOHT.
RESPONSIBILITIES:
· Support the co-design and implementation of an integrated care model to increase and optimize access to primary care in Windsor-Essex.
· Drives the engagement, participation and system co-design amongst primary care providers in Windsor-Essex.
· Provide clinical leadership aligned with the Ontario Health Team’s (OHT) purpose, priorities and values.
· Liaison between key stakeholders such as primary care providers and the OHT.
· Works in partnership with the Primary Care Sector Table and Primary Care Council to engage and represent the perspectives of clinicians during the planning and implementation of co-design initiatives that improve integrated care.
· Contribute to advancing the work of the WEOHT Primary Care Network (PCN) by working closely with the Transformation Lead, Primary Care Council (PCC), Primary Care Sector Table (PCST), Patient and Family Advisory Committee (PFAC), and Steering Committee in leading and/or participating in prioritized OHT meetings and system design sessions.
· Other ad hoc duties as assigned.
SKILLS:
· Ability to navigate challenging, crucial conversations and manage conflict resolution during the life cycle of projects and other priorities.
· System thinker with understanding of clinical leadership in health system integration, design, planning and implementation.
· Relentlessly focused on ensuring patient/client and caregiver partners have a central role in design, planning and decision making.
· Excellent communicator, strategic thinker, and team player with a passion for health system improvement and the ability to influence change.
· Self-directed and highly motivated; able to handle and navigate clinical and operational ambiguity.
QUALIFICATIONS:
· Practicing primary care physician holding a valid certificate of registration and in good standing with the College of Physicians and Surgeons of Ontario (CPSO).
· Actively practicing within Windsor-Essex.
TRAVEL:
· Valid Ontario driver’s license with access to own vehicle. Travel may be required throughout the Windsor Essex OHT region.
OTHER:
· Compensation based on WEOHT physician leadership rates. Time allocation is estimated to be the equivalent to one half day per week with additional opportunities for involvement based on need and interest.
