EMS

Essex Windsor EMS

Essex-Windsor EMS provides ambulance service and pre-hospital emergency medical care to nearly 400,000 residents of Essex County, Windsor and Pelee Island.

ESC LHIN Home and Community Care

Erie S. Clair LHIN Home and Community care serves approximately 40,000 patients annually across the Erie St. Clair Region. This includes care coordination, information and referral, and placement services. Home and Community care supports an integrated approach to service delivery and care coordination and utilizes a patient-centred approach, fundamental to the care and services provided. Family Respite Services

Family Respite Services works with families in Windsor/Essex County who are caring for individuals under the age of 18 who have intellectual, physical, or mental
health challenges. We work with families to develop an individualized, flexible plan for respite, to establish in home or out of home respite through the use of volunteers, paid in home respite providers and associate families. We also provide information about community respite options such as Assistance for Children with Severe Disabilities (ACSD) and coordinate supports for Special Services at Home for Windsor-Essex. Hospice of Windsor Essex

The Hospice provides care at no cost in our two Hospice Residential Homes (Windsor and Erie Shores/Leamington), through the Community Palliative Medicine
Program and through our Wellness Programs and Services. With only partial government funding, it has been through the generous support of our community that Hospice has become the first palliative hospice village in North America. Over time, The Hospice has adapted to meet the needs of the community, and continues to address the physical, social, emotional and spiritual needs of patients and their families, now and into the future. Life After Fifty

Life After Fifty is a charitable non-profit organization and designated Seniors Active Living Centre providing programs and services for adults fifty and over. We have been a consistent source of seniors programming in our community since 1962. Regular weekly programs and services fall under three core categories: Health and Wellness (“Be fit!”), Skills Development and Hobbies (“Be well!”), and Personal Connections (“Be social!”). We provide Community Support Services including friendly visiting, telephone assurance, foot care and home maintenance supporting at-risk older adults living at home. Pozitive Pathways Community Services

Pozitive Pathways is a registered charity that provides support, education and outreach services for people living with, affected by, or at-risk of HIV and or Hepatitis C Virus (HCV). Our services span the Windsor-Essex and Chatham-Kent counties.

Since 1988, Pozitive Pathways (formerly the AIDS Committee of WIndsor) has evolved and diversified to address the emerging needs of our community. Our staff, peers and volunteers provide in-centre and outreach services to clients and service users through our Support Service and Needle Syringe programs. Additionally, we provide prevention education and resources to community members and service providers working with populations that are most vulnerable to HIV and or HCV transmission. Hôtel-Dieu Grace Healthcare

At Hôtel-Dieu Grace Healthcare, we are committed to improving the health and wellbeing of the Windsor-Essex community through the delivery of patient-

Windsor Regional Hospital

Windsor Regional Hospital is the 11th largest hospital in the Province of Ontario and the 3rd largest community teaching hospital, serving a population of about
400,000 people in Windsor and Essex County along with providing tertiary and quaternary care to thousands more. Brentwood Recovery Centre

The Brentwood Recovery Home is dedicated to providing compassionate care and treatment in a residential setting for people whose primary problem is alcohol

Bulimia Anorexia Nervosa Association (BANA)

As the primary regional source for the prevention and treatment of eating disorders, BANA has been the leader in the promotion and acceptance of diverse body shapes and sizes through the adoption of a healthy, balanced lifestyle since 1983. Intake and Health Promotion services are provided without age restrictions with clinical services provision to the adult population. Canadian Mental Health Association (WECB)

CMHA Windsor Essex is the lead provider and advocate of specialized community mental health and addiction services integrated with comprehensive primary

Children First

Children First provides services for families with children up to six years of age who are at risk for or experiencing mental health challenges or developmental concerns. We use a family-centred approach based on the principles of respecting every family’s beliefs, values and culture. Our program respects the strengths and resourcefulness of all families and aims to support and encourage families in their efforts to meet the needs of their children. We believe in inclusion and work together with other community agencies, early learning and child care centres and schools to allow every child to have every opportunity for any future. Erie-St. Clair Clinic/RAAM Clinic Windsor Essex

This physicians community-based clinic is a regional addiction centre offering support and attention to the broader determinants of health; such as poverty
and isolation. The Clinic is part of the local health integration network partnership. The Clinic is a centre of excellence in addictions medicine with six (6) physicians attending the clinic, including two (2) female physicians serving Essex County, in Windsor and Leamington. Family Services Windsor Essex

Family Services Windsor-Essex (FSWE) is a non-profit, charitable organization serving Windsor and Essex County. We help people to restore their ability to choose

House of Sophrosyne

The House of Sophrosyne, founded in 1978, is a lead agency providing a continuum of evidence-based programs and services to women and families who are affected by the disease of addiction. The House is a designated provincial resource offering both residential and community-based addiction and trauma treatment. The organization’s name comes from Greek mythology: Sophrosyne was the goddess of discretion, temperance, and moderation. Maryvale

Maryvale is an Adolescent Mental Health Treatment Centre , where adolescents experiencing very serious emotional, psychological and mental distress can receive therapy and assistance from a team of experts. We help young people (aged 13 to 17) and their families who feel highly anxious, depressed, suicidal, worthless and hopeless, or who have lost their willingness to care about others. Services include Windsor Regional Hospital acute care beds for children and youth, day treatment, outpatient counselling and access to child psychiatrists. Learn more about Maryvale

Mental Health Connections

Mental Health Connections endeavours to empower persons (16 years of age and older) with mental health/addiction concerns to maximize their growth and potential through the use of recovery strategies. Intake Appointments are required to assess the need for program placement and to discuss desired outcomes. Programs are offered by a multi-disciplinary team, including both professional as well as trained peer support staff. Psycho-social Rehabilitation classes include: many components of fitness/nutrition; personal growth and recovery strategies; an eight week wellness program and vocational training. Amherstburg Family Health Team

"The Amherstburg Family Health Team provides comprehensive patient centred primary health care including chronic disease management, health promotion and disease prevention to the rostered patients of the Amherstburg area. The Amherstburg Family Health Team utilizes an interdisciplinary team of health professionals working in a collaborative care model. Essex County Nurse Practitioner-Led Clinic

The Essex County Nurse Practitioner-Led Clinic meets the needs of its patients/clients through timely, accessible, welcoming, knowledgeable, responsive and holistic approaches throughout the life span by the same inter-professional team and in collaboration with others who may be needed. Harrow Health Centre Family Health Team

Lakeshore Community NPLC

The Lakeshore Community Nurse Practitioner-Led Clinic provides excellent primary health care in a caring, compassionate and professional manner, promoting physical, psychological and social wellness of the individual. 

Leamington & Area Family Health Team

We are a primary health care oragnization that includes a team of family physicians, nurse practitioners, chiropodist, dietitian, nurse educators, pharmacist, social workers along with nursing and reception staff at 5 sites in Leamington and Kingsville to provide primary health care to our community. As a team, we work together with our patients to reach their goals. Our services include regular health check ups, medication reviews, footcare, chronic disease management, counselling, memory clinic, smoking cessation and nutrition counselling just to name a few. Windsor Essex Community Health Centre

As a community health centre, weCHC continuously aims to support the community of Windsor and Essex County by offering equitable, safe, quality care that is client centered and close to home. We operate six locations throughout Windsor and Essex County which include: Diabetes Wellness, Leamington, Pickwick, Street Health, Sandwich and Teen Health. In addition to these locations, we offer many services at community partner’s locations across Windsor and Essex County. Windsor Family Health Team

The Windsor Family Health team has two main programs, Family Practice and Team Care Centre. Both employ an interprofessional group of health care practitioners, including family physicians. We provide health care in an integrated, respectful and holistic environment to help keep patients healthy before illness occurs. Team Care supports complex patients, on a time limited basis, by referral from community physicians. Learn more about the Windsor Family Health Team

Windsor Regional Family Health Organization

The Windsor Regional Family Health Organization is a team of solo and group practice family physicians working together with their patients to deliver accessible, continuous, comprehensive primary health care services to the whole person with a focus on illness prevention for all ages and stages of life. Many of our physicians are actively involved in hospital in-patient care, in-patient rehabilitation care, nursing and retirement home care, and care in the patient’s home. We provide contracted services for social workers and a dietician for our rostered patients. Care is provided during regular and extended office hours and patients have access to a nurse staffed Telephone Health Advisory Service after hours who will provide them with symptom based health advice and information. THAS nurses have access to an on-call physician from the WRFHO and when appropriate will call the physician for advice regarding callers’ concerns. John McGivney Children's Centre

The John McGivney Children's Centre provides comprehensive paediatric therapy services including assessment, treatment, consultation, design and provisions of
assistive devices, education and family support services within a multi-disciplinary setting. Depending on the need, services are provided in the Centre, in the home and in our community.
