Connecting People to Home Care Services

On October 4th, 2023, the Ontario Government announced advancing Ontario Health Teams to provide people with better and more convenient connections to at-home services. If passed, the Convenient Care at Home Act, 2023, would integrate Home and Community Care Services organizations into Ontario Health atHome. Ontario has committed $128.2 million to provide every Ontario Health Team with $2.2 million over three years for better coordination of care. An additional $10.3 million will be invested to support OHTs to implement better ways to coordinate the care for patients with diabetes, heart failure and chronic obstructive pulmonary disease (COPD). Digital and virtual care options for connecting with providers online will be included in these investments.
