All Providers Better Communication Skills (ABC’s) for Seriously Ill Patients and Loved Ones

All Providers Better Communication Skills (ABCs) is a free, virtual education program being offered to all inter-professional health care providers across Canada. The ABCs program provides you with practical tools, tips and strategies to improve your communication skills when communicating with seriously ill patients and their loved ones. 

ABCs program details:  

  • Total time commitment is about 12 hours over a 3-month period.
    • 7 online modules and 3 interactive virtual workshops  
    • 3 simulated patient encounters (virtual, about 15 mins each) 
    • 3 on-line surveys 
  • Registration is from September 2024 to November 2024
  • Program begins in February 2025 or September 2025 
  • We are offering an incentive of $100 for participants who complete the full education program.  

There are limited spots available for the ABCs program because it is currently being evaluated as research. To enroll in this education, you need to first complete the registration form, an online survey, and a simulated encounter. 

**The deadline to register for the ABCs is Monday November 4, 2024.

Ontario Breast Screening Program: Expanding to People Aged 40 to 49 October 8th, 2024

The provincial-wide Ontario Breast Screening Program (OBSP) was launched in 1990 to encourage people in Ontario to get screened for Breast Cancer. As of October 8th, 2024, people ages 40 to 49 will be included in the OBSP.

***A REFERRAL FROM A PRIMARY CARE PROVIDER IS NOT REQUIRED FOR THE OBSP. PEOPLE CAN SELF-REFER BY CALLING AN OBSP SITE TO MAKE AN APPOINTMENT- SEE LOCATIONS BELOW****

OBSP screens two different groups of people in Ontario:
1) those at average risk
2) those at high risk of developing breast cancer

Goals of the OBSP:

  • Find cancer early when it is less likely to have spread to other parts of the body
  • ensure that all eligible participants receive benefots of organized screening

OBSP (AVERAGE RISK) ELIGIBILITY CRITERIA

As of October 8th, 2024, Women, Two-Spirit people, trans people and nonbinary people ages 40 to 74 are eligible for screening through OBSP if they:

  • have no new breast cancer symptoms
  • have no personal history of breast cancer
  • had not had a masectomy
  • have not had a screening mammogram within the last 11 months
  • if transfeminine, have used feminizing hormones for at least five years in a row

The Windsor-Essex Ontario Health Team is comprised of over 40 healthcare organizations, community support services, hospitals, home and community care providers, lived experience partners, and primary care providers providing services in urban and rural settings.

We are seeking a part-time 0.1 FTE Clinicial Lead (Primary Care) to be part of this dynamic Ontario Health Team

This position reports to the Steering Commitee for Windsor Essex Ontario Health Team (WEOHT).

JOB SUMMARY:

The Clinical Lead (Primary Care) works closely with key stakeholders in Windsor-Essex to support the evolution of system and resource integration within the WEOHT.

RESPONSIBILITIES:

· Support the co-design and implementation of an integrated care model to increase and optimize access to primary care in Windsor-Essex.

· Drives the engagement, participation and system co-design amongst primary care providers in Windsor-Essex.

· Provide clinical leadership aligned with the Ontario Health Team’s (OHT) purpose, priorities and values.

· Liaison between key stakeholders such as primary care providers and the OHT.

· Works in partnership with the Primary Care Sector Table and Primary Care Council to engage and represent the perspectives of clinicians during the planning and implementation of co-design initiatives that improve integrated care.

· Contribute to advancing the work of the WEOHT Primary Care Network (PCN) by working closely with the Transformation Lead, Primary Care Council (PCC), Primary Care Sector Table (PCST), Patient and Family Advisory Committee (PFAC), and Steering Committee in leading and/or participating in prioritized OHT meetings and system design sessions.

· Other ad hoc duties as assigned.

SKILLS:

· Ability to navigate challenging, crucial conversations and manage conflict resolution during the life cycle of projects and other priorities.

· System thinker with understanding of clinical leadership in health system integration, design, planning and implementation.

· Relentlessly focused on ensuring patient/client and caregiver partners have a central role in design, planning and decision making.

· Excellent communicator, strategic thinker, and team player with a passion for health system improvement and the ability to influence change.

· Self-directed and highly motivated; able to handle and navigate clinical and operational ambiguity.

QUALIFICATIONS:

· Practicing primary care physician holding a valid certificate of registration and in good standing with the College of Physicians and Surgeons of Ontario (CPSO).

· Actively practicing within Windsor-Essex.

TRAVEL:

· Valid Ontario driver’s license with access to own vehicle. Travel may be required throughout the Windsor Essex OHT region.

OTHER:

· Compensation based on WEOHT physician leadership rates. Time allocation is estimated to be the equivalent to one half day per week with additional opportunities for involvement based on need and interest.

WEOHT Trans Health Clinical Skill Development – Thank you

Centretown Community Health Centre’s Trans Health Clinic is offering capacity building to primary health care organizations/clinics interested in developing and consolidating clinical skills in the delivery of Trans health care, specifically hormone initiation and surgery referrals, as well as the identification of existing resources for leveraging wrap-around supports.

The organizing committee would like to extend their heartfelt appreciation to the attendees of the WEOHT Trans Health Clinical Skill Development Conference, held virtually on May 30th, 2024. Special thank you goes to the community members who shared their lived-experience perspective at the conference.

The morning of the conference, CCHC’s THC shared their knowledge on Trans Health in the Champlain Region (Ottawa) and provided an introduction to gender-affirming practices. The afternoon was dedicated to a Patient Journey Mapping Exercise where groups were organized to collaborate on identifying needs, gaps and resources for the Trans Community in Windsor-Essex. Active participation and engagement provided valuable insights and suggestions. These contributions will be reviewed and considered as next steps in a regional plan, which will be submitted to the Windsor-Essex Ontario Health Team for endorsement. The WEOHT will be sending to four primary care providers to CCHC for Champion development and peer-to-peer support.

CCHC’s THC is proud to have received the Health Care Innovation Award in 2023 from the Alliance for Healthier Communities for this work with The Ottawa Hospital, and The Elisabeth Bruyere Hospital. This regional sharing opportunity moves towards improving Windsor-Essex’s ability to adequately provide life-saving care and better meet the growing needs of this underserved, marginalized population.

CMHA-WECB: Over $1.9 Million for Primary Care Teams in Windsor-Essex

The Canadian Mental Health Association – Windsor-Essex County Branch (CMHA-WECB) will be a part of the Ontario government’s investment of $110 million to connect up to 328,000 people to primary care teams. $1,979,761 will be given to CMHA-WECB to expand primary care access in Windsor Essex, funding human health resources and extending the Mobile Medical Support (MMS) team outreach. The Mobile Medical Support services include:

  • Primary care check-ups (e.g. prescriptions, referrals)
  • Chronic disease support (e.g. chronic obstructive pulmonary disease, congestive heart failure, diabetes)
  • Preventative care (e.g. vaccinations, fecal immunochemical test, Pap test, mammogram referrals, health teaching)
  • Testing (e.g. Covid-19, sexually transmitted infections, pregnancy)
  • Wound care
  • Foot care
  • Mental health support
  • Substance/addiction services (e.g. overdose prevention, harm reduction supplies)
  • Community connections (e.g. housing assistance, forms, Ontario Works, social supports) 

“This investment by the Ontario government under the leadership of Premier Ford and Minister Jones, will significantly grow our primary care capacity in Windsor-Essex and serve nearly 8000 additional residents in our community,” said Andrew Dowie, Member of Provincial Parliament for Windsor–Tecumseh.

Back in February, 2024, it was announced Essex County Nurse Practitioner-Led Clinic (ECNPLC) will be receiving $424,525 to expand team-based primary care services to Kingsville, ON. These lead agencies will collaborate with the Windsor-Essex Ontario Health Team partners and WEOHT Primary Care Network to successfully meet their targets.

ECNPLC: Team-Based Primary Care Funding Announcement

The Ontario Ministry of Health announced on February 1st, 2024, that the Ontario government is investing $110 million to connect up to 328,000 people to primary care teams. This investment is a step closer towards closing the gap for 1.3 million people not connected to primary care. Visit here for full announcement.

Interprofessional primary care teams connect people to health professionals (physicians, nurse practitioners, registered and practical nurses, physiotherapists, social workers etc.) that work together to provide timely access to primary care leading to faster diagnosis, treatment and improved health outcomes.

Anthony Leardi, MPP for Essex, announced that the Essex County Nurse Practitioner-Led Clinic (ECNPLC) will be receiving $424,525 to expand its team-based primary care services. The ECNPLC has partnered with the Town of Kingsville and TMC Urgent Care Clinic to provide primary care services to the residents of Kingsville. The Ministry of Health and Ontario Health released the Primary Care Networks in Ontario Health Teams: Guidance Documents. The purpose of this guidance is to outline the vision, objectives and a common set of functions for Primary Care Networks (PCNs) in OHTs to develop over time.

MMS update: October 2023

Mobile. Medical. Support. (MMS) is a community liaison team of healthcare professionals looking to help clients in High Priority Communities (HPC) overcome barriers that prevent them from seeking healthcare and linking into community resources. After being on a brief pause while highly skilled healthcare professionals were recruited and trained to continue supporting our community’s diverse needs, MMS will again be travelling throughout Windsor-Essex to bring mobile healthcare and access to partnering agencies and social supports to HPC throughout the region. With access to primary care check-ups, preventative care (ie. cancer screening, vaccinations), mental health support, substance/addiction services, and community connections (ie. housing assistance, Ontario Works), MMS clients receive specialized care excellence that supports their overall health and wellness. All services are available to clients of all ages on a walk-in basis, free of cost. OHIP not required.
