WEOHT Trans Health Clinical Skill Development – Additional Education Opportunities

The Conference Planning Committee would like to extend their heartfelt appreciation to the attendees of the WEOHT Trans Health Clinical Skill Development Conference. Active participation and engagement have provided valuable insights and suggestions to help support the Windsor-Essex Trans Community. These contributions will be reviewed and considered as next steps in a regional plan, which will be submitted to the Windsor Essex Ontario Health Team for endorsement.

The WEOHTTransHealth Clinical Skill Development does not stop here. We have additional peer-to-peer educational presentations/knowledge transfers for primary care organizations/clinics, led by Centretown Community Health Centre’s Trans Health Clinic leads. Objectives:
-To be able to provide an affirming medical experience for a Trans person.
-To be able to recognize gender incongruence and dysphoria.
-To be able to offer safe, informed feminizing medical transition.
-To be able to offer safe, informed masculinizing medical transition.
-To be able to offer safe, informed medical transition options to non-binary folks.

Community Service/Social Service Knowledge Transfer(June 13th 9-11am) https://forms.office.com/r/GEDvsEQwLK
Peer-to-peer led by CCHC’s Brianna Holland, Social Worker
Objectives:
-Overview of gender inclusive terminology and pronouns
-Introduction to the issues and needs of Trans and non-binary patients
-Knowledge of the social determinants of health of Trans and non-binary patients
-Introduction to gender affirmative practices, including language, care and spaces
-Resource navigation for social supports and wrap-around care in the Champlain region

Peer-to-peer, Led by CCHC’s Nefertina Lau, Operations Coordinaor in Primary Care
Objectives:
-Understand the role and flow that medical reception contributes to maintaining best practices/patient safety
-operationalizing gender affirmative practices with electronic medical records
-Knowledge of gender affirmative practices for in-person, phone & e-mail interactions
-Knowledge of gender affirmative practices for environment

WEOHT Trans Health Clinical Skill Development – Thank you

Centretown Community Health Centre’s Trans Health Clinic is offering capacity building to primary health care organizations/clinics interested in developing and consolidating clinical skills in the delivery of Trans health care, specifically hormone initiation and surgery referrals, as well as the identification of existing resources for leveraging wrap-around supports.

The organizing committee would like to extend their heartfelt appreciation to the attendees of the WEOHT Trans Health Clinical Skill Development Conference, held virtually on May 30th, 2024. Special thank you goes to the community members who shared their lived-experience perspective at the conference.

The morning of the conference, CCHC’s THC shared their knowledge on Trans Health in the Champlain Region (Ottawa) and provided an introduction to gender-affirming practices. The afternoon was dedicated to a Patient Journey Mapping Exercise where groups were organized to collaborate on identifying needs, gaps and resources for the Trans Community in Windsor-Essex. Active participation and engagement provided valuable insights and suggestions. These contributions will be reviewed and considered as next steps in a regional plan, which will be submitted to the Windsor-Essex Ontario Health Team for endorsement. The WEOHT will be sending to four primary care providers to CCHC for Champion development and peer-to-peer support.

CCHC’s THC is proud to have received the Health Care Innovation Award in 2023 from the Alliance for Healthier Communities for this work with The Ottawa Hospital, and The Elisabeth Bruyere Hospital. This regional sharing opportunity moves towards improving Windsor-Essex’s ability to adequately provide life-saving care and better meet the growing needs of this underserved, marginalized population.

The WEOHT is welcoming Centretown Community Health Centre (CCHC)’s Trans Health Clinic to share the opportunity for Trans health care education and champion training. Primary Care Providers, Health Care Workers and Community Members are invited to the WEOHT Trans Health Clinical Skill Development Conference.

CCHC’s Trans Health Clinic is offering capacity building to primary health care organizations/clinics interested in developing and consolidating clinical skills in the delivery of Trans health care, as well as identifying resources for leveraging wrap-around supports. Additional presentations will be available for providers, community social service workers and administrative staff. The CCHC has the capacity to train four primary care providers (MDs, NPs, RNs) in champion training. Join the conference to learn more about future opportunities.

This regional sharing opportunity moves towards improving Windsor-Essex’s ability to adequately provide life-saving care and better meet the growing needs of this underserved, marginalized population.
