WEOHT Primary Care Network
What is a Primary Care Network (PCN)?
The WEOHT Primary Care Network (PCN) is working to connect, integrate, and support primary care providers across Windsor-Essex. Its ultimate goals are to improve the coordination and delivery of care for patients and enhance the experience of primary care providers.
The PCN has two primary objectives, as it relates to its work within the WEOHT:
1. to organize the local primary care sector for participation in WEOHT planning and provide a strong, coordinated voice in WEOHT decision-making; and,
2. to support the implementation of local and provincial health system priorities.
Through its collective voice, the PCN can help break down health system barriers, addressing inequities in health outcomes & access in local communities that will improve patient experiences. To learn more about Primary Care Networks in Ontario Health Teams visit ontario.ca .
